Saliva Collection Step-by-Step Outline:

1. Open package and place the SalivaBio Oral Swab (SOS) into your mouth, using the package to prevent touching the swab.
2. Hold the swab under your tongue for up to 2 minutes (or as long as instructed).
3. Using your mouth, place the swab into the barrel of a syringe with the plunger removed.
4. Open your storage vial and put the tip of the syringe into the vial.
5. Replace the plunger and firmly press until the swab is compressed and saliva is forced into the vial (you may repeat steps 1-5 if additional volume is required)
6. Close the vial and store as recommended or according to your instructions.

Oral Swab Cautions:

1. Use only as directed.
2. Store out of the reach of children. This device is not a toy and is intended for collection of saliva.
3. Do not use this device for children under the age of six (6), as the device may be a choking hazard for children.
4. Do not disassemble or pull apart device; discard if disassembled.
5. To ensure safety and proper use, a copy of the full protocol must be distributed to each device user.
